UIDAI accepts several "Proof of Identity", "Proof of Address" and "Proof of Date of Birth" documents for AADHAAR card change requests. Below you will find a nationally valid list of supported AADHAAR card change request documents.
Please note, you only need to send document for which you intend to make AADHAAR card changes.
Depending upon the AADHAAR card change request, a self attested supporting document will needed.
To change Name in AADHAAR card a Proof of Identity document is required.
To change Address in AADHAAR card a Proof of Address document is required.
To change Date of Birth in AADHAAR card a Proof of Date of Birth document is required.
Clearly write your AADHAAR number on all document copies.
Your name must be clearly written below the signatures/thumbprint when self attesting the documents.
For children below five years, parent/guardian can fill and sign/provide thumbprint in the AADHAAR card change request form and supporting document copies.
Small kids may not have any address proof; in that case submit your own address proof document copy for them.

my wife had applied for aadhar card wherein I have introduced as Head of family. As I have attested, no enclosures of DOB or others were not provided. However, there is a mistake in the DOB shown in my wife's aadhar card. How do I correct this now? My wife's birth certificate has name along with initials whereas in aadhar card the initials has been expanded. ( this has been brought in by the first name, last name concept). I think you should add the Birth certificate for minors in the POI
