AADHAAR Card Documents Required for Changes

UIDAI accepts several "Proof of Identity", "Proof of Address" and "Proof of Date of Birth" documents for AADHAAR card change requests. Below you will find a nationally valid list of supported AADHAAR card change request documents. Please note, you only need to send document for which you intend to make AADHAAR card changes. Depending upon the AADHAAR card change request, a self attested supporting document will needed.

  • To change Name in AADHAAR card a Proof of Identity document is required.
  • To change Address in AADHAAR card a Proof of Address document is required.
  • To change Date of Birth in AADHAAR card a Proof of Date of Birth document is required.

Clearly write your AADHAAR number on all document copies. Your name must be clearly written below the signatures/thumbprint when self attesting the documents.

For children below five years, parent/guardian can fill and sign/provide thumbprint in the AADHAAR card change request form and supporting document copies. Small kids may not have any address proof; in that case submit your own address proof document copy for them.

Dear sir/mam, my name is Harsh Verma, and my wife's name is Aditi Verma. My wife's aadhar card no (UID) is nnn5 nnn6 nnn8. The aadhar card is lost and the registered mobile number and email id are old and not in use because of which i am not able to retrieve the aadhar copy from internet. We are also not in a position of the enrolment number. So both the ways to retrieve the aadhar from internet are blocked. My son 26 years old but he is a special child ( mental disability) . In his aadhar card his 'surname' has been misspelt. He does not have any document listed under 'name change' or identity proof.
